Devuty City Manager, Special Proiects CONSENT CALENDAR DATE: February 13, 1996 SUBJECT: CALIFORNIA COMMUNITY WELLNESS FOUNDATION HEALTH PARTNERSHIP GRANT LETTER OF SUPPORT DEPARTMENT: City Manager's Office/Special Projects BACKGROUND The City of Santa Clarita has been working on a Collaborative Grant Application to the California Wellness Foundation, a non-profit foundation funded by the Health Net Corporation. The City has been successful in being selected as one of 29 finalists to complete a full request for proposal. The Health Partnership Grant initially provides a one year planning grant in the amount of $135,000. If the collaboration is successful in developing a Preventative Population Health Program and Implementation Plan during the planning period, the Wellness Foundation will fund implementation over a four-year period in the amount of $1 million, Preventative Population Health Programs are broad-based prevention strategies which are designed based on a comprehensive health assessment and public participation process to identify community health determinants. Programs may include education strategies, literacy, parenting, employment, public policy revisions, transportation and recreation in addition to actual health care services. The Request for Proposal process requires that each participating member of the collaboration submit a letter of agreement and support. A draft of such a letter from the City of Santa Clarita to the California Wellness Foundation is attached to the staff report. It is recommended that the City Council authorize the Mayor to sign a letter of support for participation by the City of Santa Clarita in a Community Collaboration under the California Community Wellness Foundation's Health Partnership Grant Program. Barker California Wellness Foundation 6320 Canoga Avenue, Suite 1700 Woodland Hills, CA 91367 Dear Ms. Barker: This letter serves as evidence that the City of Santa Clarita will serve as lead agency in the Health Partnership Program. We intend to work together in a collaboration and supportive relationship, as established by The California Wellness Foundation Health Improvement Initiative Health Partnership Program. As a participating agency in this effort, it is our intent to authorize to serve as our representative on the Health Partnership Program Steering Committee. We are committed to the concept of preventive population health programs and a comprehensive Community Population Health Assessment. The opportunity to participate in a comprehensive effort to develop and integrate population health programs is essential and we are committed to its success. A formal Memorandum of Understanding will be entered into upon grant award (projected for June 1996) and contract approval. Changes or Modification to this Letter of Support shall be made by mutual agreement by both parties. This Letter of Support maybe terminated by either party by issuing a Notice of Termination which includes the reasons for termination.
